Special Needs

Children with Special Health Care Needs (CSHCN)

Lincoln County Health Department offers families of children with special health care needs access to a Public Health Nurse for care coordination.  The Public Health Nurse can assess both the child's and the family's needs, help plan for medical care, and assist in finding resources and obtaining services.  The Public Health Nurse can meet with the family in their home or other chosen location or can consult with the family over the phone if they prefer.  There is no financial eligibility requirement for these services and there is no charge.

Eligibility:

Children must be under the age of 18 and with a condition that may prevent them from growing, developing, or playing like other children, or that places them at risk of becoming disabled or handicapped.

Financial Services:

The CSHCN program may be able to assist you with funding for needed medical services.  All other funding sources must be explored first.

An early intervention program that works to identify 0-3 year olds with special needs or developmental issues so that they receive early access to interventions that will help them reach their full potential.
